New Beneficiary Travel Claim Form

VA has created a new form for Veterans to use when submitting a claim for Beneficiary Travel mileage reimbursement, VA Form 3542 (Veteran/Beneficiary Claim for Reimbursement of Travel Expenses). This form will improve the process for requesting travel reimbursement as well as the time to process the claims.

How to Submit a Claim

The new form may be obtained at any VA medical facility or the form may be downloaded from http://www.va.gov/vaforms/medical/pdf/vha-10-3542-fill.pdf. The completed form and any required receipts may be submitted to your local VA medical facility for processing as follows:

  • Via drop-boxes located throughout the medical facility
  • Mail the form to your local VA health care facility
  • Submit via kiosks located at the VA medical facility

Claims should be submitted no later than 30 days after the appointment date. Once the claim is approved, the reimbursement check will be mailed or electronic payment will be deposited into the Veteran’s account that VA has on file.

Veterans may contact their local VA Medical facility agent cashier to notify VA of the bank account to have the reimbursement electronically deposited.
