How to Apply for Benefits Delivery at Discharge

To apply for the pre-discharge claims program, transitional military personnel should call or visit the nearest BDD site 60 to 180 days prior to their separation or retirement. It is highly recommended that interested claimants not delay in making initial contact.

Applicants must complete and submit VA Form 21-526. This form may be obtained by:

  1. Clicking here (Please note that many of the forms downloaded from the VA Forms site may not open properly with Adobe Acrobat Reader 5.0. You may find it necessary to use Adobe Acrobat Reader version 4 or version 5.05 and higher)

  2. Visiting a local BDD office.

  3. Calling 1-800-827-1000.

Program participants should arrange to have a copy made of their entire service medical records. Claims cannot be processed without them.

It is highly recommended that transitional personnel contact the Transition Assistance Coordinator for their base and attend the briefings provided by VA.

Transitional personnel may also call or visit the nearest BDD office for more information.
