Homeless Program
The Health Care
for Homeless Veterans Program is a specialized
team comprised of a coordinator, a social worker,
a nurse practitioner and a program assistant.
Staff is augmented by Americorps Volunteers. Services
include outreach, case management, Liaison and
advocacy services, supportive services, mental
health screening, medical coordination and counseling.
The Homeless Program works with community providers
and oversees placements at the Homeless Veterans
Program at Barbers Point.
