We hope you find registration for health care quick and easy. To help you with the enrollment process, we provide you with the following information:
- You can enroll by visiting the second floor of the Spark M. Matsunaga VA Ambulatory Care Center. Hours of enrollment are Monday through Friday, from 0730 am to 15:00pm (Close on Federal Holidays).
- You can also visit any of our Community Based Outpatient Clinics.
- Bring proof of honorable discharge from the military (DD Form 214)
- As part of the enrollment process, veterans are interviewed, and registration information is entered into the VA database.
- A veteran's indentification card will be mailed
to the address provided by you.

All veterans who were honorably discharge from the military service are eligible for VA health care. There is a two-year duty time requirement for veterans who served after September 8, 1980. Veterans with no service connected disabilities and no other special eligibility will be asked to provide financial information. The Department of Veterans Affairs
is required to verify income information. If you choose not disclose financial information, you may be required to make co-payments, or may be considered to be ineligible for healthcare.
Some veterans are required, by Public Law, to pay co-payments for some services. The benefits advisor will explain eligibility, and whether may be a required co-payment. A separate handout is giving with information about current co-payment rates. They can also be provided upon request at the Registration desk in all VA facilities. When you apply for VA health care, you will be asked if you have any health insurance. The VA is required by law (Public Law 99-272/Public Law 101-508) to recover from individual insurance carriers, the government's cost of providing medical treatment to veterans for non-service connected conditions. The VA has the right of recover the cost of medical care from any health insurance contract providing coverage for you, including policies held by you, your spouse, your guardian, etc.
