Advance Directives:
Information on Living Wills and Durable Power of Attorney for Health Care, commonly called Advanced Directives, is provided to new patients as they enroll with VA and as they are admitted as inpatients.  If you have any questions about Advance Directives, feel free to discuss them with your primary care provider or the social worked assigned to your Primary Care Team.
