Grant and Per Diem Progam: Per Diem Rate

Maximum Per Diem Rate is $41.90.

To ensure timely approval of rate requests, providers should read the Instructions before submitting the form.

IMPORTANT:
Per diem rate increase requests will NOT be approved until the GPD provider has submitted the SF 425 for every fiscal year since inception or FY 2004, whichever is sooner. Any questions may be e-mailed to the VA Grant and Per Diem Program.
