Who are care givers and why are they so important?

VA aims in introducing care coordination are to:
- Help ensure veteran patients receive the right care in the right place at the right time.
- Make their home (place of residence) the site where veteran patients receive care whenever this is appropriate.
- Take away barriers of distance, time and travel from veteran care.
To achieve these aims VA's care coordination programs are using telehealth technologies.
