in community nursing homes at VA expense for qualifying veterans. Under the
Veterans Millennium health care law 106-117, community nursing home care may
be provided to veterans who are in need of such care for a service connected
disability and have a service connected disability rated at 60% or more. If

funding is available for a short term contract. The residents under contract
are visited every 30 days alternately by the Nurse and Social Work
Coordinators for the program. This team also assists with discharge
planning, follow up appointments, monitoring of the quality of care
delivered to the resident in the Contract facility. In addition the CNH
