and Clinical Center (GRECC)

The
GRECC nurse is a professional nurse who assists in the design, implementation
and evaluation of clinical demonstration projects to improve the care of older
adults. She is a master’s
prepared nurse with expertise in case management, communication among team
members, education of clinicians, patients, and families, and evaluation of
models of care through clinical research. She also mentors graduate level
nursing students who are trainees of the GRECC. Students train in both the hospital (at the Geriatric Evaluation and
Management Clinic and in the GRECC Continuity of Care Project) and in the
community (making house calls with Home Based Primary Care and on the GRECC
Mobile Geriatric Unit). The mobile unit is a 31-foot vehicle outfitted with an
exam room, a conference room and a wheel-chair lift. It is used two days weekly
to provide primary care to underserved veterans in distressed inner-city
neighborhoods.
