A clinician caring for an elderly female veteran in the GRECC van which travels into town weekly to administer healthcare to those who cannot come to the VA.The GRECC nurse is a professional nurse who assists in the design, implementation and evaluation of clinical demonstration projects to improve the care of older adults.  She is a master’s prepared nurse with expertise in case management, communication among team members, education of clinicians, patients, and families, and evaluation of models of care through clinical research.  She also mentors graduate level nursing students who are trainees of the GRECC.  Students train in both the hospital (at theGRECC team members standing in front of the Mobile Geriatric Unit Geriatric Evaluation and Management Clinic and in the GRECC Continuity of Care Project) and in the community (making house calls with Home Based Primary Care and on the GRECC Mobile Geriatric Unit).  The mobile unit is a 31-foot vehicle outfitted with an exam room, a conference room and a wheel-chair lift.  It is used two days weekly to provide primary care to underserved veterans in distressed inner-city neighborhoods.
