Stereotactic Pallidotomy for Treatment of Parkinson's Disease
Policy issue: There is
a significant number of veterans with Parkinson's disease (PD)
in need of treatment. In 1998, Veterans Integrated Service Network
9 requested guidance on the use of stereotactic pallidotomy
to treat PD in the veteran population.
Assessment: VATAP produced
a systematic review of the effectiveness of stereotactic pallidotomy
for treatment of Parkinson's Disease.
Current status: In 2001,
these recommendations contributed to establishing a national
consortium of six VHA Parkinson's Disease Research, Education
and Clinical Centers (PADRECC). Each PADRECC is a participating
site in a VHA cooperative study comparing two different anatomic
sites used in bilateral deep brain stimulation in patients with
refractory PD. Originally, pallidotomy was an investigational
arm of the study, but it was dropped once literature accumulated
favoring deep brain stimulation.
