Disease Management, Depression

Disease Management is a collaborative care model for the management of patients with depressive disorders (major or minor depressive disorders, dysthymia, depression nos) who are actively enrolled in primary or medical subspecialty care. The model incorporates the use of a Behavioral Health Specialist who provides expertise in mental health assessment and is well versed in the delivery of algorithm-based management strategies for each of these disorders. The role of the BHS is to facilitate treatment, as well as to provide psychoeducation in a manner that is consistent with the Agency for Health Care Policy and Research (AHCPR) guidelines for depression. The Behavioral Health Specialist can use the BHS Floating Interview in the BHL software to track patients at any time. The BHS Floating Interview assesses depression symtoms with the PHQ-9, antidepressant use and possible side effects, alcohol use and motivation, and upcoming health care plans.
