The Joint Commission Public Notice

Do you have an issue concerning patient care and/or patient safety at the Canandaigua VA Medical Center or the Rochester Outpatient Clinic that has not been resolved by the Point of Contact, Clinical Manager, and/or Patient Advocate? If so, you may wish to contact the Medical Center’s Senior Management:

If your quality of care or patient safety concern remains unsolved after contacting the management of Canandaigua VA Medical Center, which includes the Rochester Outpatient Clinic, you may wish to contact the Joint Commission by mail, fax, or e-mail. Please summarize the issue in one or two pages and include the name, street address, city, and state of the health care organization involved. When submitting an issue to the Joint Commission about an accredited organization you may either provide your name and contact information or submit your concern anonymously.

Providing such information enables the Joint Commission to inform you about the actions taken in response to your concern and to contact you should additional information be needed. It is the Joint Commission’s policy to treat your name as confidential and not to disclose it to any other party. However, it may be necessary to share the issue with the subject organization in the course of complaint resolution.

Canandaigua VAMC Employees: The Joint Commission standards, VHA’s National Center for Patient Safety and VAMC Canandaigua do not condone taking retaliatory actions against employees for having reported quality of care or patient safety concerns. Please note that this does not supersede any requirements for reporting allegations of misconduct or violations of safety or quality of care to the Medical Center Director.
