The VA Western New York Healthcare System (Buffalo Facility) has an American Society of Health-System Pharmacists (ASHP) Accredited Pharmacy Practice Residency. The Buffalo VA Medical Center is a 200 bed Tertiary Care facility affiliated with the State University of New York at Buffalo and located adjacent to the Medical Complex. Pharmacy Service at the VA Western New York Healthcare System is affiliated with the State University of New York (SUNY) at Buffalo School of Pharmacy as a training site for doctoral pharmacy students in various aspects of Pharmaceutical Care.

Program Description

The resident will gain a variety of practice experiences including:

  • Acute/Critical Care Pharmacotherapy
  • Antibiotic Management
  • Pharmacokinetic consults
  • Research
  • Psychopharmacy
  • Geriatrics
    Ambulatory care including:
    • Anticoagulation Clinic
    • Dyslipidemia Clinic
    • Diabetes Clinic
    • Geriatrics
    • Long Term Care
Learning Experiences

During the course of the program, the resident will be expected to:
  1. Provide Acute/Critical Care Pharmacy Services, including total parenteral nutrition, antibiotic preparation, antibiotic monitoring, pharmacokinetic consults, IV admixture and Unit Dose Drug Distribution, participation in medical and surgical patient care rounds, provision of discharge counseling, and provision of Drug Information Services
  2. Provide Primary Care Clinical Pharmacy Services including patient /provider education, continuity of care and pharmacotherapy services and various medical specialties including: Dyslipidemia Management, Anticoagulation, Cardiology, Diabetes, Hypertension, Dermatology, Mental Health, Geriatrics, Critical Care and Infectious Disease.
  3. Demonstrate effective written and oral communication skills with physicians, nurses, and other health-care practitioners. Documentation of written skills should be in the form of written progress notes or formal consultations in the patient's chart and interdepartmental memoranda.
  4. Design, implement and analyze an IRB (institutional review board approved) research project.
  5. Develop computer skills to assist in the conduct of professional activities.
  6. Develop, implement and present a drug usage evaluation (DUE) Demonstration Project.
  7. Provide education for Pharmacy Staff, providers, nurses, other health care professionals and patients utilizing both informal and formal (videoconference) continuing education (CE) inservices.
