Payment Management System & Standard Form Resources

Transition in Place, Case Management, Special Need, and Capital grantees will need to register with the Payment Management System and set up direct deposit. Grantees should submit their SF-425 via the Electronic SF-425 Submission Portal and email the completed form with supporting documentation (general ledger) to GPD425@va.gov

Per Diem Rate Request Information:

UPDATES:

Per P.L. 116-315, the per diem rate request forms below have been modified to allow for additional costs (up to $10,000) associated with HMIS.

State Home Domiciliary rate increased to $54.89; therefore, the maximum per diem rate authorized, per the waivers under the CARES Act and P.L. 116-315, is $164.67 for transitional housing and $20.58 for service centers. NOTE: When the current per diem waiver ends the maximum per diem rate is expected to be $63.12.

Homeless Veterans Dental Program (HVDP):

The HVDP promotes oral health for all Veterans that have faced, or are at risk for, homelessness. Veterans who are enrolled in eligible residential rehabilitation programs may be eligible for dental care. Please find further information in this pamphlet:

Fair Housing ACT & Resources:

The Fair Housing Act protects people from discrimination when they are renting or buying a home, getting a mortgage, seeking housing assistance, or engaging in other housing-related activities. Additional protections apply to federally-assisted housing.

LGBTQ+ Resources:

There is an LGBTQ+ Veteran Care Coordinator (LGBTQ+ VCC) in every VA health care system to help Veterans get the care they need. VHA policies require that Veteran's health care is delivered in an affirming and inclusive environment and that VHA employees respect their identity. The LGBTQ+ VCC can answer questions, advocate for the Veteran's right to quality care, handle complaints or concerns that Veterans have about their care, and help them get started with services for LGBTQ+ Veterans.
