The Homeless Screener Clinical Reminder (HSCR) was developed in 2012 to proactively identify those Veterans who are at-risk of homelessness or are currently experiencing homelessness.
The HSCR is completed on all Veterans enrolled in VA care when they present at their primary care appointments
