Dear SSVF Grantees,

 

Thank you for all of your work with community planning to end Veteran homelessness. We are excited to release a nationwide community planning survey today, and we are asking for your assistance. The purpose of the survey is to help us to better understand community progress related to ending Veteran homelessness. The information gathered as a part of this survey will help the VA—along with HUD and the U.S. Interagency Council on Homelessness (USICH)—tailor training and technical assistance so that it more effectively addresses local needs. We are asking you to coordinate responses with CoCs, Veteran Work Groups, the local VA Medical Center homeless program leadership, and other Community Partners. While SSVF grantees will facilitate submissions, it is critical that they are  a collective response that accurately depicts community progress and needs. SSVF grantees should not submit responses in isolation. Additionally, this is not an evaluation. Instead, it is meant to be a supportive process; therefore, honest and open responses are strongly encouraged. Survey submissions (1 per CoC) will be due on or before Wednesday, May 3rd

We have prepared several tools to assist you with this process:

We strongly recommend viewing the brief, instructional video as your first step. The video acts as a virtual Companion Guide to the survey and covers the survey purpose, expectations, and a detailed demonstration of the survey in Survey Monkey. The video can also be a good reference tool when a community is preparing survey responses if additional clarification is needed.

 

Submissions

Similar to the previous survey, we can only accept one survey submission per CoC. As a result, grantees must coordinate if they have shared geography with other grantees to determine who will submit the survey for each CoC. We understand that some geographic areas such as Balances of State are expansive. There are areas in the survey where additional comments can be added based on local needs.

 

Please do not access the survey link below unless you are designated to complete the survey and are ready to submit your answers. (Clicking on the link prematurely will result in a blank submission. To prepare responses, please refer to the PDF Version of the Survey Questions and the video.)

Please remember to select “Done” to submit the response. Please note that we recommend preparing responses in advance and then entering them into Survey Monkey in one sitting to avoid time outs with the tool. Additionally, there is no feature to save a partially completed survey and return later.  Lastly, an automatic copy of the response will not be provided upon submission; therefore, it is important to keep notes on survey responses. Once all surveys are submitted, we will be able to post pdfs of the submissions to the SSVF website.

 

Survey Deadline: All survey responses must be submitted no later than Wednesday, May 3, 2017.
