SSVF Program Change Requests: Due March 1, 2018

 

This is a reminder that the deadline for submitting program change requests to the SSVF Program Office is 3/1/2018.  The requests must be submitted using the SSVF online grants management system.  Grantees can access the Program Change Form on the “Requirements” tab of their grant account.  Please note that the form will be inactive after the deadline.  Therefore, SSVF is unable to accept submissions after this point. 

Remember, avoid the common mistakes!  Submit the necessary attachments.  Ensure that explanations for budget modifications are sufficient.  Verify that the total amount of the grant award equals the awarded amount from the VA, if there are adjustments to the HHS subaccount balances.  If Program Change Request does not contain sufficient information, the request will be denied by the Program Office.

 

New Resource: VA SSVF Coordinated Entry Fact Sheet

 

The SSVF Program Office is pleased to release the VA SSVF Coordinated Entry Fact Sheet, as a resource for SSVF grantees and partners.  The fact sheet provides a basic overview of coordinated entry (CE), how it works, and the primary benefits of each component of coordinated entry systems. Although the general benefits of coordinated entry systems are known, this resource provides specific examples that can be used as grantees and partners interact with system planning and refinement. This document also assists grantees and VA partners with the context of coordinated entry which can support their implementation of the fall 2017 VA and Coordinated Entry Memo published by VA Deputy Under Secretary for Health for Operations and Management. As a refresher, the memo provided guidance to VA medical center staff and staff of VA funded homeless assistance programs regarding their roles in supporting local Continuum of Care (CoC) coordinated entry systems (CES), as required by the U.S. Department of Housing and Urban Development. The memo from the VA to the VA medical centers is meant to support community planning and CES efforts within CoCs by clearly outlining the expectations of VA medical center involvement. In many ways, this guidance codifies what has already been occurring in local communities. Where new partnerships are needed, it provides the opportunity and framework for engagement.
