The SSVF Program

Resolution in their communities. The next SSVF Rapid Resolution Office

New: Rapid
Resolution HMIS Data Entry Revised Guidance
The SSVF Program
Office has received many comments from SSVF providers voicing their concerns
about the Rapid Resolution HMIS data entry requirements. These
expressed concerns combined with new SSVF screening and enrollment guidance
have prompted the SSVF Program Office to make changes to how and when SSVF
providers should conduct Rapid Resolution conversations with Veterans.
Effective immediately, the SSVF Program Office recommends that Rapid Resolution
conversations only be conducted with Veterans who have been enrolled in SSVF
RRH or HP projects. This new guidance eliminates the need for
grantees to enter Rapid Resolution conversations and the results of those
conversations into a separate Coordinated Entry Event HMIS
project. While the SSVF Program Office will no longer be
tracking the numbers of Rapid Resolution conversations in HMIS; it is expected
that grantees will attempt Rapid Resolution with the majority of Veterans who
are seeking SSVF assistance. Detailed information about these
changes will be presented during the November 22 Rapid Resolution Office Hours
call and included in the updated Rapid Resolution Compliance Guide when it is
released later in November.
Reminder:
End of Year Closeout Requirements
Per SSVF
regulations, grantees are required to complete end of the year program closeout
activities. All grantees have received an email notification
providing instructions for completing the FY2019 SSVF Grant Closeout
Requirements. Grantees have 45 days from the end of the FY2019 grant term to
complete these end of year activities. Grantees can review the end of
year requirements by reviewing the September SSVF National Grantee webinar that
provided an overview of the closeout requirements. The SSVF Program
Office has also provided a companion guide to assist grantees in the completion
of the FY2019

Reminder:
SSVF Contact Change Reporting Process
SSVF Contact
Changes are reported to the SSVF Program Office using the SSVF online grants
management system (GIFTS). Grantees will find the SSVF Contact Change
Template in Applicant’s Account (GIFTS Account holder) GIFTS account portal as
an active Requirement, titled SSVF Contact Change Form. To access current
contact list, grantee may open up the SSVF Contact Change Requirement and
current contacts will be prepopulated. IF NO CHANGES ARE NEEDED, PLEASE DO NOT
SUBMIT. If changes are needed, Grantees will fill out the form with
new contacts and submit. Please complete and submit the form as
needed. The SSVF Program Office will provide a new, blank form for
future use.
