NEW: Emergency Housing Assistance (EHA) and COVID-19 Vaccine Tracking due by COB Thursday, July 8, 2021 (PDF attached)

The SSVF Program Office is requesting updated EHA data as well as COVID-19 Vaccine Tracking data for the month of June 2021 from all grantees. This information must be submitted through Survey Monkey by COB Thursday, July 8, 2021.  The survey questions are attached in this Program Update and the survey may be assessed through this link: https://www.surveymonkey.com/r/59PYQPM. Please reach out to your Regional Coordinator if you do not receive the survey or have any questions.

REMINDER: Data Entry for Services Provided

Service provision is an integral component of the SSVF Program.  As a reminder, SSVF grantees are required to complete HMIS data entry for services provided to enrolled Participants.  This includes Health Care Navigation, Rapid Resolution, Legal, etc.  For further information about  SSVF Supportive Services requirements please reference the SSVF Program Guide on the website at: SSVF_Program_Guide.pdf (va.gov). For additional details about recording Services Provided, please reference the VA Data Guide at:  VA Data Guide – FY 2020; Specifically, Section V. SSVF Reporting, Services.csv. 

 

 

REMINDER: Fraud, Waste and Abuse Compliance

It is incumbent on an SSVF awarded organization to report all instances of potential or actual fraud as soon as your organization is made aware of the issue. Notifying your Regional Coordinator and the Program Office is an essential part of keeping in compliance with the executed Memorandum of Award (MOA) as outlined in Sections 2, 4, 7, 8, 15 and 20. GOOD NEWS: Health Care Navigator – Restart Inc.

Shannon Dimaggio is a Health Care Navigator with SSVF program, Restart Inc., in Kansas City, MO who has been supporting Veterans with establishing care at the Kansas City VA Medical Center (VAMC).  Recently, she met with a Veteran who was not comfortable with his assigned primary care provider, and as a result, had not been seen by his provider in a number of years.  Learning of this specific concern, and noting the Veteran’s need for care, Ms. Dimaggio educated the Veteran on how to request a new provider and receive the services necessary to address his whole health needs.  Because Ms. Dimaggio provided a clear path for the Veteran to follow in seeking this reassignment, the Veteran has now been scheduled to see his new primary care doctor at the Kansas City VA Medical Center.  As an SSVF Health Care Navigator, Ms. Dimaggio advocated for the Veteran and communicated with VA staff to ensure the Veteran was allowed to request a new provider, be scheduled for an appointment, and have a plan in place to address any issues that arise prior to being seen.  Great work Shannon Dimaggio!  As others experience highs and lows with your work as a Health Care Navigator, please share them with your Regional Coordinator.
