- I am a clinical trialist with an interest in the effect of bariatric surgery and adrenalectomy on metabolism and homeostasis. I am interested in two areas. The first relates to disorders of the adrenal gland. Generally, these include disorders of the adrenal cortex, specifically aldosteronomas leading to primary aldosteronism and excess cortisol production. Primary aldosteronism leads to an increased risk of stroke and renal disorders. Excess cortisol production has myriad complications which include loss of bone mass, diabetes, weight gain, and increased mortality. We are trying to understand opportunities around streamlined identification of these disorders as well as the impact of surgical therapy on short and long-term complications. We will be using VINCI to identify Veterans with hypertension on multiple medications who have a diagnosis of primary aldosteronism and determine from that cohort of those who have unilateral disease. We will create two cohorts which we will follow retrospectively to ascertain improvements in blood pressure, renal function, development of stroke and mortality, among others. Second, we proposed to evaluate patients who are candidates for weight loss surgery and determine their outcomes relative to those who either receive glucagon-like peptide receptor agonists or nonsurgical therapy. We will better understand implementation of lifestyle modification using VINCI. Finally, we proposed to conduct a randomized clinical trial to determine if sleeve gastrectomy can achieve improvements in liver inflammation for those patients with metabolic associated steatohepatitis compared to those who undergo best medical management.
