Notice to Veterans: The VA Health Information Exchange Opt Out Form, VA Form 10-10164, is now available online at www.va.gov, on this website, or in the Release of Information Office at local VA medical centers.  The form must be signed, mailed and/or hand carried to a Veteran's local Release of Information Office.  Currently, there are no means of submitting the forms electronically.  

VA will begin opting all Veterans into health information sharing, in or after January 2020.


Do not want to share your electronic health information (Opt Out):


Online:
 
Currently, we are unable to process VA Form 10-10164 electronically

Mail:

1. Download and print the VA form 10-10164 not to share (Opt Out)
2. Mail it to “Release of Information Office” at your local VAMC

In Person:

1. Visit the Release of Information (ROI) Office at your local VAMC.
2. Ask for and complete VA Form 10-10164 (Opt Out)
3. Give the form to a member of the Release of Information office staff.

Changed your mind and now want to share your electronic health information, after you Opted Out (Opt back In):

Online:
 
Currently, we are unable to process VA Form 10-10163 (Opt back in) electronically

Mail:

1. Download and print the VA form 10-10163 to share (Opt Back In)
2. Mail it to “Release of Information Office” at your local VAMC

In Person:

1. Visit the Release of Information (ROI) Office at your local VAMC.
2. Ask for and complete VA Form 10-10163 (Opt Back In)
3. Give the form to a member of the Release of Information office staff.

For those who are currently sharing their electronic health information, if at any time you wish to stop sharing your information, visit your local VA medical center’s Release of Information Office and ask for a “stop sharing” form (VA Form 10-0484) or call 1-844-MyVA311 (1-844-698-2311), select "0", and ask for Release of Information (ROI) Office. However, if you submit VA Form 10-0484, before December 30, 2019 your preference will remain honored when VA systems start sharing with providers who treat you in or after January 2020, and no further action is needed by you.


Are you a resident of a Dual Consent State (ex:  MN and NY, etc.,)? 


PLEASE NOTE:  Signing up to share your health information with VA may not complete the process in allowing both VA and your Community Provider to exchange your health records.   There may be cases in which providers, state laws and/or Community Care Provider(s) require an additional Consent Form in order to exchange information with the VA.  Please contact your local Release of Information (ROI) Office and/or local Community Care Provider(s) to confirm whether an additional consent form is needed for your area.
