Homeless Outreach engages/connects homeless Veterans to healthcare, needed housing, and other supportive services. For more information:

 

HCHV Contract Residential Services Program provides short-term housing and supportive services to eligible Veterans who need immediate housing as they work toward permanent housing. For more information:

 

Grant and Per Diem (GPD) provides transitional housing and treatment to eligible Veterans. For more information:

 

Housing and Urban Development – Veterans Affairs Supportive Housing (HUD-VASH) program provides eligible homeless Veterans with a Housing Choice rental voucher and wrap around supportive case management services to assist with obtaining housing stability. For more information:

 

Health Care for Reentry Veterans (HCRV) program provides short term case management to address the needs of incarcerated Veterans to prevent a return to homelessness when re-entering into the community. Also provides assistance with referrals and linkages to medical, mental health and social services, including employment services on release For more information:

 

Veterans Justice Outreach (VJO) assists eligible justice-involved Veterans encountered by police, and in jails or courts, with obtaining timely access to VHA mental health, substance use, and homeless services. For more information:
