The Dayton VA Hospice and Palliative Care unit has an annual memorial to remember Veterans who have passed over the last year.
The Dayton VA Hospice/Palliative Care Program serves Veterans with a serious or life-limiting diagnosis, and their family members, to improve quality of life. The Dayton VA Hospice/Palliative Care team strives to provide peace and comfort to those we serve during this difficult time in their lives as well as after the Veteran’s death. Bereavement support is offered to the surviving family of Veterans who pass away at the Dayton VA Medical Center for up to 13 months following the Veteran’s death.
How to Get Hospice/Palliative Care
If you or your loved one are interested in Hospice or Palliative Care, please discuss with your Patient Aligned Care Team (PACT). Your PACT Provider may place a consult for the Hospice/Palliative Care team to establish care.
