Report a Privacy Complaint
A privacy complaint is a verbal or written statement to a Privacy Officer regarding an alleged privacy violation. Privacy violations involving VA Sensitive Information should be reported as soon as possible in person, by mail, email, phone, or through My HealtheVet Secure Messaging. Be sure to provide as much detail as possible about the violation including:
- Your Name & Contact Information
- Date of suspected privacy violation
- Who was involved in the privacy violation?
- What happened? (E.g., You received another Veteran’s information in the mail.)
Record Amendments
Request a Record Amendment
An amendment request of a VHA Health record must be submitted in writing and clearly describe the record to be amended. This includes identifying the specific record or note title, date of the record or note, and the specific information within the record or note requested to be amended. Amendment requests may be submitted in person, by mail, email, or through My HealtheVet Secure Messaging.
Freedom of Information Act (FOIA)
Submit a FOIA Request
A FOIA request must be made in writing and reasonably describe the agency records that are being requested. FOIA requests for non-personal, Fargo VA Health Care System records, must be submitted in person, by email to vhafarfoia@va.gov or mailed to: Fargo VA Health Care System, FOIA Office 001P, 2101 Elm Street North, Fargo , ND 58102. Please note that medical records are requested through the Release of Information Department.
