SSVF Participant Satisfaction Survey
Per the SSVF Final Rule, grantees are mandated to provide each exiting participant household with a SSVF Participant Satisfaction Survey. In order to satisfy this requirement, grantees should electronically distribute the survey to participants within 30 days of the participant's exit from the program, using a customized weblink provided by VA contractor, M. Davis and Company (MDAC).
In accordance with 38 CFR 62.63, each grantee is subject to a VA monitoring visit. The purpose of the monitoring visit is to ensure the grantee is in compliance with regulations described within 38 CFR part 62. This comprehensive toolkit will assist SSVF providers in preparing for monitoring visits by guiding grantees through the process and conducting a monitoring self-assessment. The toolkit includes a Uniform Monitoring Package (UMP) Crosswalk that can be used to help grantees understand questions on the UMP and where to find additional information about each question.
The SSVF Program Office processes a majority of SSVF grant requirements within the GIFTS online grants management system.
