VA employees, VA contractors, and any other HIPAA-covered individuals or entities (e.g., private healthcare providers and insurance providers) should not submit personally identifiable information, protected health information (PHI), or other VA-sensitive information (including patient names) via this web submission form. You may include multiple attachments. The attachments must be a PDF, text or image file no larger than 4 megabytes. Acceptable file extensions are gif, jpg, jpeg, png, doc, docx, key, pdf, pps, ppsx, ppt, pptx, odt, xls, xlsx, rtf, and txt. Attachment Instructions

If you need to select files from different folders on your computer, FIRST copy the files that you want to upload into the same folder on your computer, then follow the above steps.

By choosing to remain anonymous, the OIG cannot obtain additional information regarding the allegation (e.g., testimonial or documentary evidence; identity of witnesses), and also cannot inform the complainant as to what action VA OIG has taken on the complaint.
