Entitlement to service connection for a respiratory disability other than post-surgical residuals of pulmonary nodules, to include bronchitis, chronic obstructive pulmonary disease (COPD), bronchiectasis, and pulmonary embolism is remanded. REASONS FOR REMAND The Board remanded the claim in March 2016 and June 2017. As noted in the last remand, according to a November 2016 VA Form 27-0820 (Report of General Information), the Veteran stated that he did not received the June 2016 Supplemental Statement of the Case (SSOC). The Board instructed this be sent to him. This was not done. The June 2017 Board remand stated there was evidence that the Veteran was in receipt of long term private disability benefits and requested that any relevant records be obtained. While a letter with a VA Form 21-4142 was sent in June 2017, the Veteran did not respond. The Veteran sent a letter dated in November 2017 reporting he had moved. Based on the last time he verified his address, the Veteran moved at some point between November 2016 and October 2017. No documents have been returned as undelivered. It is unclear what, if any, correspondence the Veteran received after November 2016. The relevant correspondence includes the Board June 2017 decision and the request for any private disability benefits or other private treatment records. The August 2018 SSOC in the file is for the wrong Veteran. The matters are REMANDED for the following action: 1. Provide the Veteran with copies of the June 2016 SSOC and the June 2017 Board remand. 2. Request that the Veteran either provide any relevant records related to his private disability benefits, to include private medical records, or complete a release for such records. If any records requested are not ultimately obtained, notify the Veteran pursuant to 38 C.F.R. § 3.159(e) (2017). 3.
