Entitlement to a disability rating in excess of 30 percent for recurrent headaches is remanded. Entitlement to an initial disability rating in excess of 30 percent for right carpal tunnel syndrome (CTS) is remanded. REASONS FOR REMAND 1. Right Shoulder 2. Headaches 3. Right CTS In a May 2016 letter, Dr. K. stated he sees the Veteran every one to two weeks primarily due to his increasing pain and loss of function related to the disabilities on appeal. In several statements of record, the Veteran indicated the significance of this treatment, however, he did not submit a formal request for VA to assist him in obtaining these records. Given their potential relevance, a remand is required to allow VA to obtain authorization and request these records. The matters are REMANDED for the following action: Ask the Veteran to complete a VA Form 21-4142 for Dr. K. Make two requests for the authorized records from Dr. K., unless it is clear after the first request that a second request would be futile.
