REASONS FOR REMAND The Veteran served on active duty from September 1983 to September 1997. Evidence indicates that there may be outstanding relevant VA and private treatment records. The Veteran testified that he sought sinus and nasal related treatment at the James Haley VA Hosptial in Tampa Florida after his separation from service. The Veteran further tetified that he received two nasal surgeries from private practioners to treat his sinus condition. A remand is required to allow VA to obtain these relevant records. The matter is REMANDED for the following action: 1. Obtain the Veteran’s VA treatment records for the period from October 1997 to April 2007. (Continued on the next page)   2. Ask the Veteran to complete a VA Form 21-4142 for physicians and facilities where he received nasal surgery from private practitioners.
