G.V. (Sonny) Montgomery VA Medical Center in Jackson, Mississippi is looking for Registered Nurses (RN) for the Community Living Center (CLC)!

The Registered Nurse (RN) for the Community Living Center (CLC) utilizes the nursing process (assessment, diagnosis, outcome identification, planning, implementation, and evaluation) and provides care to residents admitted in the CLC. He/she is responsible for planning holistic comprehensive person care focusing on an interdisciplinary approach to ensure the resident attains or maintains the highest practical physical, mental, and psychosocial well- being. He/she maintains expertise in skilled nursing care, rehabilitation, restorative care, long term care and hospice and palliative care. He/she possess knowledge of the Center for Medicaid/Medicare Services Federal Regulations for Long-Term Care and the use of the Resident Assessment Instrument (RAI) Minimum Date Set (MDS). The RN is responsible for performing assessments, developing consistent assignment, facilitating Watch List Huddles, participating in quality assurance performance improvement, resident and family education, admissions, discharges, transfers, record audit, nursing procedures and medication administration. The primary practice setting is in the CLC. The CLC is a component of the spectrum of long-term care that provides a skilled nursing environment and houses a variety of specialty programs for persons needing short and long stay services which include skilled nursing, rehabilitation, continuing care, restorative care, and hospice and palliative care.
