Health Care for Homeless Veterans (HCHV) Program: is a vital VA initiative focused on addressing homelessness among Veterans. It oversees several specialized subprograms that work collaboratively to provide support. These programs emphasize proactive outreach to identify and engage Veterans experiencing homelessness and connect them with comprehensive services through both VA and community-based resources. The subprograms are listed below in more detail.

  • Homeless Employment Services: Support Veterans experiencing homelessness in securing and maintaining meaningful employment. These services include job readiness training, resume and interview preparation, and referrals to vocational programs and employers. Employment specialists work closely with Veterans to identify career goals and overcome barriers to employment. By promoting financial independence, these services play a key role in supporting long-term housing stability and successful community reintegration.
  • Grant and Per Diem Program (GPD):  A transitional housing initiative that partners with community-based organizations to provide supportive housing and services to Veterans experiencing homelessness. The program offers time-limited housing along with case management, clinical support, and assistance with employment and independent living skills. GPD is designed to help Veterans achieve residential stability, increase their income, and obtain permanent housing.

  • St. Anne’s Transitional Home for Soldiers (Diamond Bar) (men) – POC: Gail Moore, Program Director/Owner,

  • Contract Residential Services Program:  Provides short-term, community-based residential housing for health care-eligible Veterans experiencing homelessness who require a stable and structured environment. Veterans may stay in the program for up to three months, during which they receive support tailored to their individual needs. These placements are designed to assist Veterans in varying stages of change as they work toward permanent housing. The program offers additional support to help Veterans engage in treatment, stabilize their health, and pursue personal goals in a safe and supportive setting.

  • Life Community Development (Loma Linda) POC – Joseph Perkins -
  • Community Outreach: is focused on identifying and engaging Veterans experiencing homelessness in the community. Outreach staff conduct regular visits to shelters, encampments, streets, and other locations where unhoused Veterans may be found. The goal is to build trust, assess needs, and connect Veterans to VA health care, housing resources, and supportive services. This outreach is often the first point of contact for Veterans who are disconnected from services, and it plays a critical role in reducing barriers to care, promoting early intervention, and guiding Veterans toward long-term stability and housing.
  • Veterans Justice Program: is a prevention-focused initiative within the VA’s Homeless Programs Office. Its mission is to identify and support Veterans involved in the criminal justice system—particularly those in local jails or courts—by connecting them with VA services before they become homeless or further entangled in the justice system. VJO Specialists work closely with law enforcement, court systems (including Veterans Treatment Courts), and community partners to ensure that justice-involved Veterans have access to mental health care, substance use treatment, housing resources, and other supportive services. The program aims to reduce recidivism, promote recovery, and support successful community reintegration.
  • Health Care for Reentry Veterans (HCRV) The Health Care for Reentry Veterans (HCRV) Program is designed to assist incarcerated Veterans as they prepare to transition from correctional facilities back into the community. The program focuses on outreach, assessment, and linkage to essential VA services such as health care, housing, and mental health or substance use treatment. HCRV aims to reduce the risk of homelessness and recidivism by ensuring Veterans have access to the support they need upon release.
  • Housing and Urban Development – Veterans Affairs Supportive Housing (HUD-VASH): Program is a collaborative permanent housing initiative between the VA and local Public Housing Authorities across the Inland Empire. It serves Veterans and their families who are experiencing homelessness by providing clinical case management, housing assistance and subsidies through HUD, employment support, peer support services, and nursing care through the VA. This integrated approach ensures that Veterans not only secure stable housing but also receive the comprehensive support needed to maintain long-term housing stability and improve overall well-being.*The Veteran must be currently homeless, as defined by HUD criteria, and must meet income guidelines of the designated Public Housing Authority.
  •  HUD-VASH Nursing Team: are an integral part of the HUD-VASH Program, providing direct clinical support to Veterans as they transition from homelessness into permanent housing. These services are delivered by registered nurses who work closely with case managers to address the physical and mental health needs of Veterans enrolled in the program. Nursing staff conduct health assessments, provide medication education and management, coordinate care with VA medical providers, and support chronic disease management. The goal of VASH Nursing Services is to enhance overall health outcomes and support long-term housing stability through integrated, Veteran-centered care.
    • HUD-VASH Occupational Therapy Team:  Occupational Therapy (OT) and Occupational Therapy Assistant (OTA) services within the HUD-VASH Program are designed to support Veterans in achieving greater independence and stability as they transition into permanent housing. These services focus on enhancing daily living skills, promoting healthy routines, and addressing barriers that may impact a Veteran’s ability to maintain housing and overall well-being. OT practitioners assess each Veteran’s functional abilities and develop individualized plans to improve areas such as home management, budgeting, medication organization, community navigation, and coping strategies. OTAs work under the supervision of licensed occupational therapists to implement these plans through hands-on support and skill-building activities.
