This category is appropriate for complainants who fear reprisal for contacting the OIG. The OIG will protect the identity of complainants to the greatest extent possible. In rare instances, the OIG may disclose your identity if such disclosure becomes unavoidable during the course of its work. The OIG will attempt to contact you before releasing your identity. Please provide personal contact information to help the OIG comply with nondisclosure preferences.

You may include multiple attachments. The attachments must be a PDF, text or image file no larger than 4 megabytes. Acceptable file extensions are gif, jpg, jpeg, png, doc, docx, key, pdf, pps, ppsx, ppt, pptx, odt, xls, xlsx, rtf, and txt. Attachment Instructions

The OIG will protect the identity of complainants to the greatest extent possible.
