These services are provided to Veterans who are assessed and admitted to the various programs, which include Health Care for Homeless Veterans (HCHV), the Grant and Per Diem (GPD) program, the Veterans Justice Outreach (VJO) program, Homeless Patient Aliened Care Team (HPACT), and Housing and Urban Development (HUD) Veteran Affairs Supported Housing (VASH) program. We can also connect you with resources in your community.
