VA recognizes Milwaukee VA National HeRO Awardee for commitment to patient safety

Milwaukee , WI — The U.S. Department of Veterans Affairs announced today it has honored Milwaukee VA Medical Center Registered Nurse Jeremy Sauld with a National HeRO Award for his contributions to the Veterans Health Administration’s Journey to High Reliability.

In 2021, Sauld discovered certain syringes used for IV lines throughout the hospital were defective. Specifically, after an IV line was prepped, the barrel of the syringe would automatically retract, creating an air bubble in the line and the syringe.

Sauld worked with Supply Chain Management to get the defective syringes off the shelves. He also submitted his findings to the FDA, which contributed to a recall of some 267 million syringes throughout the country. (Learn more here.)

VHA's Journey to High Reliability aims to continuously improve its processes by maximizing patient safety and reducing harm to restore trust and ensure every Veteran receives the high-quality care they deserve.

“I am immensely proud of the work Jeremy has accomplished,” Milwaukee VA Medical Center Director Dr. Daniel Zomchek said. “Receiving a HeRO Award means our staff members have embraced High Reliability principles and practices by continuously improving our processes to ensure VHA delivers the best care possible. Veterans have a champion on their side when it comes to Jeremy.”

The National HeRO Award is the highest level of HRO recognition available within VHA and is reserved to honor employees who advance VHA’s Journey to High Reliability through demonstration of VHA’s HRO Principles in action.
