Returning Service Member Care with the Military 2 VA (M2VA) Program

The Phoenix VA Health Care System’s M2VA Program addresses the transition and readjustment needs of Service Members and Veterans as they transition from Active Duty military service and the DoD health care system to the VA health care system and the next phase of the Veterans' lives. Care may include: transitional care management, initial VA vesting medical appointments, mental health services with a focus on post deployment and readjustment related concerns, mental health medication evaluation and management, assistance navigating through the VA Healthcare System, and additional resources.
