Geriatric Primary Care
Geriatrics Primary Care is an outpatient visit for primary care for elderly Veterans. A VA team provides coordinated care that includes medical care, nursing care, therapy, mental health, disease treatment and prevention, health maintenance and education, referral for specialty care, and overall care management. The primary care providers are trained in the management of healthcare problems associated with aging and the elderly.
