Dr. Kenneth Shay, Director of Geriatric Programs at the Veteran’s Health Administration, introduces six examples of VAs home based alternatives to long-term care. The sample programs highlighted below are from nearly 150 programs introduced since 2009 in sites from Vermont to Virginia, Honolulu to Florida. The programs provide dependent veterans and those who care for them with a range of proven new choices for staying connected with their families, their communities and with the country they fought to protect.

This is a research-based home hospital program run by physicians and nurses to prevent hospitalization or get patients out of the hospital earlier (2 minutes).
VA video Dementia Compentencies at Home VA Dementia Competencies at Home
This program maintains Veterans in their home through dementia care education and support to their caregivers. Caregivers get the tools they need to keep their home safe (1.5 minutes).		 PACE (Program for All-inclusive Care for the Elderly) VA PACE Program
PACE (Program for All-inclusive Care for the Elderly) provides all inclusive care for Veterans in one location (2 minutes).
VA_video_longtermcare_integrating_care VA Integrated Care
This model, integrating geriatrics and mental health imbedded in primary care, is uniquely suited to better manage all the Veteran’s medical and psychosocial needs (1.5 minutes).		 VA_video_longtermcare_GRACE VA GRACE
A GRACE team consists of a nurse practitioner and social worker acting as a liaison with the VA primary care provider. The GRACE team meets with the patient in the home (2 minutes).
VA Home Based Transitional Care VA Home Based Transitional Care
