How to Create a VA/DoD Sharing Agreement

  • VA/DoD facility leadership decide to enter into an agreement.
  • Complete VA Form 10-1245c via MAX.GOV
  • The participating local VAMC/VISN Director and DHA/DoD Facility Director must sign the form electronically in MAX.GOV
  • Once approved, the sharing agreement is recorded in the VA/DoD MAX.GOV Resource Sharing Database and email notifications are sent.
