Geriatric Patient Aligned Care Team (Geri-PACT) offers enhanced ambulatory care expertise for managing community-dwelling Veterans whose healthcare needs are particularly challenging due to multiple chronic diseases, coexisting cognitive and functional decline as well as psychosocial factors. Home and Community Based Services including: Adult Day Health Care, Home Based Primary Care, Homemaker and Home Health Aide Care, Hospice and Palliative Care, Program of All-Inclusive Care of the Elderly (PACE), Respite Care, Skilled Home Health Care, Telehealth Care, and Veteran Directed Care.
Residential Settings including: Community Residential Settings, Adult Family Homes, Assisted Living and Medical Foster Homes.
Nursing Homes including: Community Living Centers, Community Nursing Homes and State Veterans Homes.
